Personal information is anything that identifies you as an individual such as your name, phone number, birthday, etc. Personal health information is information about your health or health care history such as a test result with your name on it.
Information gathered by the Centre is kept in confidence. Our staffs are authorized to access personal information based upon their need to deal with the information for the reason(s) for which it was obtained. Safeguards are in place to ensure that the information is not disclosed or shared more than is necessary to achieve the purpose for which it was gathered.
