If you are looking for a family physician we welcome you to consider becoming a registered client. Once you are registered with our Centre you will be able to receive primary health care services from one of our teams and be eligible to use all of the services we offer.

Why must you register?

  1. All persons who access services at Southwest Middlesex Health Centre agree to use our service providers as their main heath care provider.
  2. People who see more than one doctor (visit a walk-in clinic and/or emergency department for non-urgent reasons) increase the overall cost of our health care system.

Steps to becoming a registered client.

  1. Complete an application form.
  2. Your application will be reviewed. Priority will be given to individuals who do not have a family physician and who may have difficulty obtaining health and social services.
  3. You will take part in an intake interview where you will tell us more about your health needs and we will share with you what we can offer to you.
  4. Once you become a registered client you will be assigned to one of our medical teams.
